On October 19, 2009, Governor Corzine signed into law P.L. 2009, c.141 also known as the “Citizen Service Act.” One of the provisions of the new law requires any person interested in volunteering their services on a municipal authority, board, commission or committee to file a one-page “Citizen Leadership Form” with the Township Clerk. In completing this form, you should be aware that the law also deems several items of information on the form to be restricted from public disclosure under the Open Public Records Act. The design of the form facilitates OPRA disclosure and redaction of the restricted information by locating those fields at the bottom of the form.
Following completion of the form, please print it out and mail it to the Township Clerk with a cover letter and resume. Your cover letter should explain the reasons why you wish to serve on a particular authority, board, commission or committee. Mail all of the above to:
You may also email the information to: firstname.lastname@example.org or you may fax it to: (609) 268-7430.
